Tibial Plateau Fracture Classifications
Tibial plateau fractures are classified by the Schatzker classification. 

  1.  Lateral split tibial plateau fracture – usually in younger patients, with no depression at the articular surface
  2. Split with depression – typically older patients with osteoporosis 
  3. Pure lateral depression – without splitting through the articular surface
  4. Pure medial depression – medial tibial plateau crumbles into fragments
  5. Bicondylar – split fractures through both the medial and lateral tibial plateaus
  6. Split extends to metadiaphyseal region – severe stress fractures that result in separation of the tibial plateau from the underlying tibia 

Treatment of Tibial Plateau Fracture
Many factors influence what treatment is required for tibial plateau fractures. The factors include:

  • degree of displacement
  • number of particles of bone
  • extent of soft tissue and ligamentous injuries
  • associated neurovascular injuries
  • open/closed status of fracture

Once treatment has been decided on, it is important to get therapy. Initially the therapy will consist of early range of motion (ROM) without putting any weight on the affected leg. A realistic goal is to gain 90 degrees of flexion by 4 weeks. It is vitally important to not bear weight on the affected leg for 12 weeks.

Possible Complications from a Tibial Plateau Fracture

  • Edema
  • Loss of ROM
  • Loss of strength
  • Loss of function
  • Deep vein thromboses
  • Myositis ossificans (muscle pain accompanied by bony deposits in the muscle)
  • Pseudoarthrosis (formation of a false joint formed by fibrous tissue)
