On January 5, 2011, Triad Group, a manufacturer of over-the-counter products in Hartland, Wisconsin, initiated a recall involving ALL LOTS of ALCOHOL PREP PADS, ALCOHOL SWABS, and ALCOHOL SWABSTICKS manufactured by Triad Group due to the risk of contamination with Bacillus cereus. These products were "private labeled" for many accounts as follows:
Cardinal Health
PSS Select
VersaPro
Boca/ Ultilet
Moore Medical
Walgreens
CVS
Conzellin
If you or a family member contracted a Bacillus cereus infection after use of one of these Triad alcohol products, you may have the right to sue Triad Group and, if the product used was relabeled, the company that relabeled the product, for example Cardinal Health or Walgreens. These claims may involve the following:
- Strict liability
- Negligence
- Breach of warranty
This Triad alcohol pads and swabs recall involves products marked as STERILE as well as non-sterile products. Use of contaminated Alcohol Prep Pads, Alcohol Swabs or Alcohol Swabsticks could lead to life-threatening infections, especially in at risk populations, including immune suppressed and surgical patients.
Triad Alcohol Prep Pads, Alcohol Swabs and Alcohol Swabsticks are used to disinfect prior to an injection. The recalled products were distributed at the wholesale, hospital, and retail pharmacy levels.
They were distributed nationwide to retail pharmacies and are packaged in individual packets and sold in retail pharmacies in a box of 100 packets. The affected Alcohol Prep Pads, Alcohol Swabs and Alcohol Swabsticks can be identified by either “Triad Group,” listed as the manufacturer, or the products are manufactured for a third party and use the names listed below in their packaging:
Cardinal Health
PSS Select
VersaPro
Boca/ Ultilet
Moore Medical
Walgreens
CVS
Conzellin
If you have any of these types of products in your possession listing “Triad Group” as the manufacturer, you should not use the product
